What the standard application form is (Annex 11)
The funeral grant application is a standard form known as Annex 11 (Anexa 11), used at all pension houses. Together with the supporting documents, it is the basis on which the grant is approved.
The form has a fixed structure — you do not write anything from scratch, you simply fill in the required information.

What the form covers
The standard form asks for several categories of information:
- Deceased's details — full name, national ID number (CNP), last status (insured worker or pensioner)
- Claimant's details — full name, CNP, ID document number, capacity in which they are claiming
- Chosen payment method — cash or bank transfer
- Declaration that the claimant bore the funeral costs
- List of documents attached to the application
Who completes and signs the form
The form is completed and signed by the person who bore the funeral costs — a family member or any other person who can prove payment.
Kinship does not matter. What matters is that the signatory is the person claiming the grant and who will receive payment.
The declaration about funeral costs
The form includes a statutory declaration (declarație pe propria răspundere) that the claimant bore the funeral costs. Under certain conditions this declaration may stand in place of documentary proof of payment.
The safest approach is to keep the invoices and receipts as well — see what goes into the application file.
Where to get the form
The standard form is available at the territorial pension house and on CNPP's official website for download. It can be filled in at home before you go, or at the counter.

Deadline for filing
The application can be filed within the general limitation period of 3 years from the date on the death certificate. It does not have to be filed during the burial days.

- Wrong details for the deceased — especially the CNP
- Missing signature from the claimant
- Payment method left blank
